To make your life easy, I work with several managed care insurance companies. I will check your insurance benefits for you. If I am not a provider in your plan, most plans provide for out-of-network benefits. If you do not have insurance, affordable payments can be arranged.
I can be reached at: 407-588-7880. I check my messages constantly throughout the day, so phone calls are the best way to contact me. I will call you back as soon as I can.
However, if you prefer to email me, you can do so at: hopkinssara9@gmail.com. Please realize, however, that I may not get right back to you until the end of the day or even the next day, as I may not have the time to check my emails as often.
If we make an appointment, you can download the following forms to fill out and bring to your first appointment. That way we can start discussing your concerns right away and not have to waste time doing paperwork.
